Hounslow and Richmond Community Healthcare NHS Trust is a National Health Service (NHS) provider of community health services in west London. The trust provides a range of services, including mental health, sexual health, community nursing and therapy services. It is based in Hounslow and Richmond upon Thames, with a catchment area covering parts of the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

The trust was established in April 2011, following the merger of Hounslow Primary Care Trust (PCT) with the Hounslow and Richmond Community Health NHS Trust. It provides services to over 800,000 people living in west London. The trust employs more than 2,500 staff across 40 sites in the region.

The trust has three main areas of focus: delivering quality care for all; transforming patient experience; and making sure that care is safe and effective. This includes providing integrated care for people with long term conditions; working with communities to make sure their needs are met; developing innovative models of care; improving access to mental health support services; and providing evidence-based clinical advice across all its services.

The trust has been commended by national organisations such as the Care Quality Commission (CQC) for its commitment to quality improvement and patient safety. In 2016 it was awarded an 'outstanding' rating by CQC for its services overall, one of only two such ratings given to community trusts in England at the time. The trust continues to strive for improvement through investment in digital technology and other innovative approaches such as cloud-based systems.
