Central London Community Healthcare NHS Trust (CLCH) is a community-based health care provider in the United Kingdom. It provides services to over 1.2 million people living in the boroughs of Westminster, Kensington and Chelsea, Hammersmith and Fulham, and parts of Barnet. The trust was established in April 2010 following the merger of Central and North West London NHS Foundation Trust with West London Mental Health NHS Trust. The trust provides a range of services including district nursing, health visiting, physiotherapy, speech and language therapy, dietetics, podiatry/chiropody, occupational therapy and sexual health services. CLCH also runs a number of specialist clinics such as dermatology clinics for adults and children; audiology clinics; diabetic retinopathy screening; hearing aid fitting; adult mental health services; learning disability services; dementia support teams; drug & alcohol treatment services; smoking cessation programmes; bereavement counselling and many more. In addition to providing healthcare directly to patients through its clinical teams across the four boroughs it serves, CLCH also works closely with local GPs to ensure that patients receive appropriate care when they need it most. This includes running GP out-of-hours centres across Westminster which provide urgent medical advice when regular surgeries are closed or fully booked up. The trust has been praised for its commitment to patient safety by introducing new systems such as electronic prescribing which reduces medication errors significantly compared with traditional paper prescriptions methods used by other trusts in England. In addition to this it has implemented an innovative ‘Patient Safety Dashboard’ which allows staff at all levels within the organisation to monitor performance against key indicators such as infection rates or waiting times for appointments quickly and easily from any computer connected to the internet – helping them identify areas where improvements can be made before they become serious problems for patients or staff members alike.
