Heywood Middleton and Rochdale Primary Care Trust (PCT) was a National Health Service (NHS) organisation that provided health services to the people of Heywood, Middleton and Rochdale in Greater Manchester, England. It was established in April 2002 as part of the NHS reforms and was responsible for commissioning healthcare services from hospitals, GPs, community health services and mental health services. The PCT also had responsibility for public health initiatives such as immunisation programmes, smoking cessation campaigns and healthy eating advice.

The PCT employed over 1,000 staff across its various sites including doctors, nurses, allied health professionals and administrative staff. It had an annual budget of £400 million which it used to purchase healthcare services from providers such as hospitals, GPs and community health services. The PCT worked closely with local authorities to ensure that the needs of local people were met through effective commissioning of healthcare services.

In April 2013 Heywood Middleton and Rochdale PCT merged with other local NHS organisations to form NHS North West Boroughs Healthcare Commissioning Group (NWBHCG). NWBHCG is now responsible for commissioning healthcare services in Heywood Middleton & Rochdale as well as Bury & Bolton.
