Shropshire Community Health NHS Trust is an NHS trust based in Shropshire, England. The trust provides a wide range of community health services to the people of Shropshire and its surrounding areas. Services provided by the trust include district nursing, health visiting, midwifery, sexual health services, podiatry, physiotherapy, occupational therapy and speech and language therapy. The trust was established in April 2006 following the merger of Shropshire Primary Care Trust (PCT) and Shropshire Community Health Services (CHS). It has since grown significantly in size and scope, providing services across a number of different sites throughout Shropshire. In addition to its core services, the trust also runs a number of innovative projects designed to improve the quality of life for local residents. These include schemes to reduce falls amongst older people, as well as programmes aimed at tackling obesity and smoking cessation.
