Norfolk Community Health and Care NHS Trust (NCH&C) is a provider of community health and care services in Norfolk, England. It was established in April 2020 following the merger of Norfolk Community Health Services NHS Trust and Norwich Community Health Services NHS Trust. The trust provides integrated health and social care services to people living in the county, including mental health services, learning disability services, adult social care, children’s community nursing and therapy services.

NCH&C works with a range of partners to provide high quality healthcare for local people. These include GPs, hospitals, charities and voluntary organisations. The trust has over 3,000 staff working across more than 80 sites throughout Norfolk providing care for adults and children alike. It also provides specialist advice on mental health issues such as depression or anxiety as well as support for those with long-term conditions like diabetes or dementia.

The trust has an ambitious vision to become a centre of excellence for integrated community healthcare in Norfolk by 2021. To achieve this it is investing heavily in technology such as electronic patient records which will help improve communication between different teams within the organisation as well as allow patients access to their own medical records online via a secure portal.
