Liverpool Community Health NHS Trust is a community health trust providing health services to the population of Liverpool, England. The trust was established in April 2012 following the closure of the former Liverpool Primary Care Trust and Liverpool PCT Mental Health Trust. It is responsible for providing primary and community care services including mental health, sexual health, learning disabilities and public health services across Merseyside.

The trust covers an area of approximately 150 square miles around Liverpool City Region which includes Halton, Knowsley, Sefton and St Helens as well as parts of Warrington. It provides its services through over 70 community sites including two hospitals, five GP practices and numerous specialist clinics located in communities across Merseyside. Services are delivered by a team consisting of doctors, nurses and other healthcare professionals such as physiotherapists, occupational therapists and dietitians.

The trust strives to provide high quality patient care with an emphasis on prevention rather than cure through its range of public health initiatives such as smoking cessation clinics, physical activity programmes, weight management schemes and alcohol harm reduction programmes among others. In addition it works closely with partner organisations to ensure integrated working between all levels of healthcare service providers so that patient needs can be met more effectively while ensuring continuity of care throughout their treatment journey from initial diagnosis to post-treatment follow-up visits . This approach has been recognised by various national awards including the Health Service Journal award for ‘Excellence in Integrated Care’ in 2017/18.

As part of its commitment to improving outcomes for local people it also engages with local communities through engagement events focusing on particular topics such as diabetes or cancer prevention awareness days held at various venues across Merseyside where members can learn about these conditions from experts within their own communities . The trust also provides education training opportunities for staff who wish to further develop their skills in order to deliver better healthcare outcomes .
