Sheffield Health and Social Care NHS Foundation Trust is a provider of mental health, learning disability and community health services in Sheffield, England. It is one of the largest mental health trusts in England and Wales. The trust works closely with local partners across South Yorkshire to deliver integrated healthcare services in local communities including GP practices and other voluntary sector organisations such as Age UK Sheffield who provide advice on practical matters like benefits entitlement or assistance to keep safe at home.
