Kensington and Chelsea Primary Care Trust (PCT) was established in April 2006, following the merger of the former Kensington, Chelsea and Westminster PCTs. The Trust provides health services for a population of about 270,000 people living in the London Boroughs of Kensington and Chelsea, Hammersmith and Fulham, and Westminster.

The Trust provides a range of primary care services including general practice, community nursing, health visiting, district nursing, school nursing, mental health and learning disabilities services. It also has responsibility for commissioning secondary care services from hospitals, specialist services and community services.

The Trust is committed to providing high quality health services that are accessible to all. It works in partnership with local authorities, voluntary organisations and other health and social care providers to ensure that the services it commissions meet the needs of its population.

The Trust has set itself a number of key objectives to improve the health and wellbeing of local people. These include: improving access to primary care services; reducing health inequalities; improving the quality of care provided; increasing patient involvement in decision-making; and increasing public understanding of health issues.

The Trust is also committed to supporting public health initiatives such as smoking cessation and obesity prevention. In addition, it is actively involved in developing integrated care pathways to ensure that patients receive the right care in the right place at the right time.
